Veterinary Conditions
I, Dr ………………………………, a duly authorised government veterinary officer, hereby certify that:
Live fish, their gametes and fertilized eggs for aquaculture or rearing purpose (excluding glass eels):

(1) Live fish, or the broodstock of gametes or fertilized eggs have been kept at the aquaculture facility of origin approved by the relevant state or territory authority, for at least fourteen days immediately prior to the shipment or collection of the gametes or fertilized eggs. There has been no incident of high mortality in that specific species of fish which was caused by communicable diseases or unknown etiology in the aquaculture facility of origin during the three months prior to export or collection of the gametes or fertilized eggs.

(2) The consignment meets one of the following conditions (mark "X" as applicable):
I. The following basic biosecurity measures have been implemented at the aquaculture facility of origin for at least the previous two years:
i) The listed diseases in "Quarantine Requirements for the Importation of Live Fish and Their Gametes and Fertilized Eggs" to which the export animals are susceptible are notifiable to the competent authority of the exporting country; and
ii) Aquatic animals introduced for breeding have originated from area or aquaculture facility free from the listed diseases in "Quarantine Requirements for the Importation of Live Fish and Their Gametes and Fertilized Eggs" to which the export animals are susceptible or from aquaculture facilities where basic biosecurity measures have been implemented; and
iii) The aquaculture facility of origin has been subjected to an official fish health surveillance scheme conducted by a laboratory designated by the Australian Government according to the procedures described in the Manual of Diagnostic Tests for Aquatic Animals of the World Organization for Animal Health (hereafter referred to as the OIE Aquatic Manual) and is certified free from the listed diseases in "Quarantine Requirements for the Importation of Live Fish and Their Gametes and Fertilized Eggs" to which the export animals are susceptible.
The following diseases which the export animals are susceptible are under official surveillance complying with this condition:

II. Within thirty days immediately prior to the exportation of live fish, their gametes or fertilized eggs, samples were collected from the aquaculture facility of origin in accordance with the OIE Aquatic Manual. The samples have been tested with negative results for diseases listed in “Quarantine Requirements for the Importation of Live Fish and Their Gametes and Fertilized Eggs” to which the export animals are susceptible, by an Australian government approved laboratory and using the methods described in the OIE Aquatic Manual.

(3) The live fishes prior to export from the waters or aquaculture facility of origin, or their gametes and fertilized eggs prior to collection from the population of origin of fishes, are found to be healthy without external parasite infestation or clinical signs of animal diseases within 7 days prior to export.

(4) For the sample collection and testing, if the test methods of listed diseases are not designated in the OIE Aquatic Manual, the test methods of the diseases that have been published in international science journals were used.
