V. Information concerning the state of health of the fish/fish eggs/fish sperm and their disease free origin:

The undersigned, (INSERT NAME) the Government Veterinary Officer responsible for verification of origin, hereby certify that:

a) At the time of despatch of the fish/fish eggs/fish sperm, the consignment was not subject to any official veterinary restriction with regard to fish diseases;

b) The fish/fish eggs/fish sperm originate from an establishment which is under regular official scrutiny for the possible outbreak of fish diseases.

Points a) and b) apply to freshwater fish which are intended for breeding or for commercial purposes (livestock fish), for fish eggs or fish sperm production if the consignment originates from fish breeding establishments, fish hatcheries or the like, in addition:

c) The consignment of fish/fish eggs/fish sperm at the time of despatch was free from the following diseases:

  • infectious pancreatic necrosis of salmonids (IPN)
  • infectious haematopoietic necrosis (IHN)
  • viral haemorrhagic septicaemia (VHS)
  • Cyprinide spring viraemia

Points a) and b) apply to freshwater fish which are intended for breeding or for commercial purposes (livestock fish), for fish eggs or fish sperm production if the consignment originates from an open water system, and in addition:

d) In the water system from which the fish/fish eggs/fish sperm were taken, there is no official record of the occurrence of the following diseases:

  • infectious pancreatic necrosis of salmonids (IPN)
  • infectious haematopoietic necrosis (IHN)
  • viral haemorrhagic septicaemia (VHS)
  • Cyprinide spring viraemia

Points a) and b) above apply to freshwater fish which are to be used as food.

VI. This certification remains valid for 10 days following its date of issue; if the consignment is transported by sea, its validity is extended to cover the period of the voyage as well.

If the consignment originates from a fish breeding establishment, fish farm or the like:

  1. The consignment of fish at the time of despatch was free from the following diseases:
    • infectious pancreatic necrosis of salmonids (IPN)
    • infectious haematopoietic necrosis (IHN)
    • viral haemorrhagic septicaemia (VHS)
    • spring viraemia of carp (Cyprinide spring viraemia)

    If the consignment originates from an open water system

  2. In the water system from which the fish were taken, there is no official record of the occurrence of the following diseases:
    • infectious pancreatic necrosis of salmonids (IPN)
    • infectious haematopoietic necrosis (IHN)
    • viral haemorrhagic septicaemia (VHS)
    • spring viraemia of carp (Cyprinide spring viraemia)

Port of entry requirements

  1. The consignment must be accompanied by an official health certificate. 
  2. The certification remains valid for 10 days following its date of issue; if the consignment is transported by sea, its validity is extended to cover the period of the voyage as well. 
  3. Neither the official seal nor the signature should be rendered in black ink.

Special conditions

These conditions only apply to freshwater species.

  1. At the time of despatch of the fish, the consignment was not subject to any official veterinary restriction with regard to fish diseases.
  2. The fish originate from an establishment which is under regular official scrutiny for the possible outbreak of fish diseases.
